UIC Class V Well Inventory

This registration is for a person who owns an existing or proposed UIC Class V well. Wells must be registered and managed in myDEQ | Go to myDEQ > 

Class V UIC wells used in conjunction with golf course maintenance do not need to be registered. However, any vadose zone injection wells (including Class V UIC wells) that receive stormwater mixed with reclaimed wastewater or groundwater, or both, from manmade bodies of water associated with golf courses, parks, and residential areas must be registered. The registration is valid for the life of the Class V well, until it is plugged and abandoned

Effective October 15, 2025, the U.S. Environmental Protection Agency (EPA) will transfer primary enforcement authority (primacy) for all classes of UIC wells, including Class V, to the State of Arizona. This means that the Arizona Department of Environmental Quality (ADEQ) is now the primary regulatory agency responsible for the UIC program in Arizona.
